Y8 / Y9 Girls' HPV Vaccinations

Year 8 GIRLS Only: HPV Vaccination – Thursday 19 January 2017
Letters and forms from the Bristol Community Children's Health Partnership have been sent home to parents of all Y8 girls regarding the national HPV (human papillomavirus) vaccination programme. The HPV vaccine, to protect against the commonest cause of cervical cancer, is being offered to all Y8 girls and will be administered in school by a School Health Nurse on Thursday 19 January.
 
The School Health Nurse Team requires explicit written consent in order to vaccinate your daughter.  Please return the NHS Consent Form to the Student Participation Team (via main reception) by Friday 13 January.  
 
If you do not wish your child to receive this vaccination you must still complete and return the consent form completing the relevant box to indicate your choice.

Year 9 GIRLS Only: HPV Vaccination – Thursday 19 January 2017
For those girls currently in Y9 who had their first HPV Vaccination in school when they were in Y8, the follow-up vaccination will also be offered in school on Thursday 19 January. You do not need to complete and return a new consent form as the School Health Nurse Team already have them from the first round of vaccinations.

A timetable of vaccinations for Y8 and Y9 girls will be issued for the day and your daughter will be collected from her lesson at the appropriate time.

Please ensure your daughter attends school on this date and, where possible, please avoid booking other medical appointments, such as orthodontic visits, on this day.
